Based on years of research, the following tips can help you get the most from your surveys:
1) Ask questions in the first person, such as "I commute to work by...". We have found people like to talk about themselves more than to be interrogated.
2) Keep questions short and to the point. Ideally, questions can be read and answered in 6 to 8 seconds!
3) Ask only one question at a time! Too often, two separate questions are bundled together such as "I experience depression or anxiety..."
By keeping questions short and concise, you can ask more questions than you might expect. Add instant feedback and placing survey on your own website, and you will get better results.
